SAN J D AR I N Environmental Health Department <br /> I Y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> O'Reilly Auto Parts DC#23 3412 Perlman Dr, Stockton August 25, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 109 CCR 66262.23(x)(4) Failed to send paper originated generator manifest copies to DTSC within 30 days. <br /> OBSERVATION: Operations Manager Seth Austin did not know if generator copies of Uniform Hazardous Waste <br /> Manifests(UHWM)were sent to the California Department of Toxic Substances Control (DTSC)within 30 days of <br /> hazardous waste shipment. <br /> REGULATION GUIDANCE: Hazardous waste generators are required to send a copy of the generator signed <br /> UHWM to DTSC by mail.The only exception is when an UHWM starts and finishes as an electronic manifest in the <br /> e-Manifest System. The generator shall within 30 days of each shipment of hazardous waste submit to DTSC a <br /> legible copy of each manifest used.The mailing address is: DTSC Generator Manifests, Department of Toxic <br /> Substances Control, P.O. Box 400, Sacramento, CA 95812-0400. <br /> CORRECTIVE ACTION: Ensure that copies of all generator signed UHWMs are sent to DTSC within 30 days of <br /> shipping the hazardous waste. Also provide a corrective action statement and supporting documentation to the <br /> San Joaquin County Environmental Health Department(EHD). <br /> This is a repeat violation, Class II. <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> OBSERVATION:A spill control pallet for used oil in the Oil Storage Room contained a significant amount of liquid. <br /> REGULATION GUIDANCE:All facility spill control equipment shall be maintained as necessary to assure its proper <br /> operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the spill control pallet into proper working order by removing the liquid in the <br /> secondary containment and placing in the proper waste container.
