IFAssociates <br />MILANI <br />• Utilize manual/mechanical excavation methods to determine the location of the two septic tanks and <br />the limits of the leach field in proximity of the property boundary between Parcel A and Parcel B. The <br />septic tanks will be physically potholed to expose the corners and entrance point for each septic tank <br />and closed physical terminus of each leach line to the property boundary between Parcel A and Parcel <br />B. All potholing will be performed by the property owner <br />• Coordinate site inspection with SJC-EHD to witness the exposed limits of the septic tanks and leach <br />field for final regulatory approval. <br />• Verify using visual observation methods that there are no underground utilities present within the <br />limits of the leach field <br />• Per SJC-EHD Condition of Approval (COA) #3 -Prepare a final report that includes revised site plans <br />showing the following- <br />a. <br />ollowing <br />a. Location and configuration of the existing sewage disposal system, <br />b. Confirm by map note that no underground utilities are present within the current leach <br />field limits' <br />11he disposal (leach) field area must be used for that specific purpose only, and it cannot contain any underground utility lines (San Joaquin <br />County Development Title, Section 9-1110.4(c)(5)). <br />Work Plan Summary: <br />The work plan includes the following tasks to be performed: <br />• Task 1 Work Plan Preparation and Regulatory Permitting <br />• Task 2 SJC-EHD Inspection Coordination: Well, Septic Tank and Leach Field <br />• Task 3 Civil Engineering. Surveying <br />• Task 4 - Final Report <br />The scope for each of the above tasks are described below: <br />Task 1 - Work Plan Preparation and Regulatory Permitting <br />As part of the work plan implementation, the following documents requests will be made to the owner and to <br />San Joaquin County Environmental Health Department: <br />Historical Onsite Wastewater Treatment System (OWTS) Permit Documentation — Where the <br />historical OWTS permit documentation cannot be provided by the property owner, perform a search <br />of existing the San Joaquin County Environmental Health Department (SJC-EHD) Permit records <br />database for the prior OWTS permit submittal for the residence. Where not present on the SJC-EHD <br />electronic record database, complete and submit a Public Records Release Form to the SJC-EHD. The <br />Public Records Release request would include requesting existing files for the wastewater treatment <br />plant (septic tank and leach field).
