... & *Associates <br />MILANI <br />Record of Previous Well Completion Report - Where the historical Driller Well Completion Report <br />(WCR) documentation cannot be provided by the property owner, complete and submit a Public <br />Records Release Form to the SJC-EHD. The Public Records Release request would include existing <br />files for the onsite well. Where SJC-EHD does not have information on the well, Milani will contact <br />the California Department of Water Resources (DWR) to obtain the applicable Well Completion <br />Report. This may require a letter from the existing property owner to release the Well Completion <br />Report if a redacted copy of the well completion record cannot be obtained. <br />• Develop written workplan to address the specific work elements for each work area identified below. <br />Workplan would identify specific work practices or Standard Operating Procedures to be implemented. <br />The workplan will include the geophysical survey, regulatory inspection coordination and civil <br />engineering surveying. <br />The workplan would include preparing and submitting the SJC-EHD Service Request Form for the <br />onsite regulatory inspection. Based on discussion with SJC-EHD, the regulatory site inspection <br />is not anticipated to exceed $304. Milani & Associates will remit the regulatory inspection fee <br />payment with the submittal of the Service Request Form. <br />A copy of the permit record request is included in Appendix A. A copy of the Consultation Permit/Service <br />Request is included in Appendix A. Payment Documentation for the Consultation Permit is included in <br />Appendix B. <br />Task 2 -SJC-EHD Inspection Coordination: Well, Septic Tank and Leach Field <br />Following completion of potholing, Milani & Associates will contact SJC-EHD to perform the necessary site <br />inspection to visually observe the exposed limits of the septic tanks and the leach field. The SJC-EHD inspector <br />will also witness the location of the existing water well in relation to the septic system. <br />Tape measurements will be made to verify that the necessary setback distances for the well and property line <br />with respect to the septic system conform to the minimum required setback distances. Photographs of exposed <br />portions of the leach field and septic tank and the surface topography around the OWTS system will be taken. <br />Task 3 Civil Engineering. Surveying <br />Following completion of Task 2, Milani & Associates will survey each pothole location. The survey will include <br />the horizontal location, ground surface elevation and elevation of the exposed septic tank or leach line at each <br />pothole performed at the septic tanks and within the leach field. The surveyors will also survey the marked <br />horizontal limits of the septic tanks and leach field. The surveyors will also survey the existing water well <br />location. <br />The field survey data will be reduced in the office and the data incorporated onto existing civil engineering <br />drawings for the Tentative Parcel Map submittal.
