Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />September 01,2022 <br />Facility Address: <br />1323 W CHARTER WAY, STOCKTON <br />Facility Name: <br />ASSOCIATED TRACTOR SVC INC <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR -Notice to Comply) <br />RemarksItem # <br />11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br />OBSERVATION: Documentation showing, current, relevant initial and annual refresher training for Efrain Cardenas, <br />Bonifacio Cardenas, Santana Martinez, Miguel Torres, and Gabriel Caldera was not available. <br />REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br />receive initial and annual refresher training. All employees shall be trained in safety procedures in the event of a <br />release or threatened release of a hazardous material, including, but not limited to, familiarity with the following <br />plans and procedures: <br />(A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br />program agency. <br />(B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment. <br />(C) Evacuation plans and procedures, including immediate notice, for the business site. <br />This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br />years. <br />CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br />training records to the EHD. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection, September 1, 2022. On <br />September 22, 2022, the EHD has written this complete report which replaces the initial checklist. Complete <br />and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the <br />corrective actions that have been or will be taken for each violation, and any supporting paperwork, by October <br />22, 2022. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />of having them sign. <br />Starting January 1, 2022, all in-office CERS help will be provided at EHD hourly rate ($156).
