Geosyntec <br /> consultants <br /> Geosyntec H&S Procedures referenced herein are available on Geosyntec's H&S SharePoint site TASK HAZARD ANALYSIS <br /> and should be consulted,as appropriate,to ensure requirements are met.This THA has been (Ver.3.1,1-4-2021) <br /> prepared per"HS-204-Work-Specific Hazard and Risk Assessment,Written Safety Plans." <br /> Part A — PROJECT/TASK INFORMATION <br /> Project/Site Name: GSH OxyChem-Stockton Project Number/Org.: TR0712/8135 <br /> Site Address: 1904 West Charter Way,Stockton,California <br /> Task&Worksite Installation of Dual-Nested Soil Vapor Probes at Off-Site Location <br /> Description: <br /> A set of dual-nested soil vapor probes(SVPs)will be installed in a single borehole of ten-feet depth at the work site <br /> (located in the sidewalk adjacent to the residences at 1925 and 1917 Old Spanish Drive in Stockton).Prior to start of <br /> field work,Geosyntec will notify USA811 and will also contract a private utility locator to pre-clear the drilling location, <br /> which is planned to be in the City of Stockton's sidewalk(i.e.,right of way). The borehole for the SVPs will be advanced <br /> using hand auger only. Cuttings will be visually logged by Geosyntec staff,and undisturbed samples will be collected <br /> from 4.5-to 5-feet below ground surface(ft bgs)and from 9.5-to 10-ft logs using a slide hammer and metal sleeves. <br /> Once terminal depth is reached,the SVPs will be installed per the Addendum to the Work Plan for Vapor Intrusion <br /> Investigation. All field work will be performed under Geosyntec supervision and Geosyntec staff will perform air <br /> monitoring during all intrusive subsurface work.
