CAD 990 794 133 Modification 0 <br /> (3) Monitoring, testing or analytical data where <br /> required; and <br /> (4) A record of the amount of subsidence that has <br /> occurred over the past year. <br /> 2. AVAILABILITY RETENTION AND DISPOSITION OF RECORDS <br /> (a) All records including plans, required in this <br /> document shall be furnished upon request, and made <br /> available at all reasonable times for inspection, <br /> by an officer, employee, or representative of <br /> the Agency who is duly designated by the Director. <br /> (b) The retention period for all records required in <br /> this document is extended automatically during the <br /> course of any unresolved enforcement action <br /> regarding the facility or as requested by the <br /> Agency. <br /> 3 . ANNUAL REPORT <br /> The owner or operator shall prepare and submit a single <br /> copy of an annual report to the Agency by the anniver- <br /> sary of the effective date of this permit each year <br /> beginning 1990. The annual report shall cover facility <br /> activities during the previous calendar year and shall <br /> include the following information: <br /> (a) The EPA identification number, name, and address <br /> of the facility; <br /> (b) The calendar year covered by the report; <br /> (c) A summary of any releases; <br /> (d) A summary of site maintenance activities; <br /> (e) Monitoring data; <br /> (f) The amount of subsidence that has occurred to <br /> date; and <br /> (g) The certification signed by the owner or operator <br /> of the facility or his authorized representative. <br /> VIII. CORRECTIVE ACTION FOR CONTINUING RELEASES <br /> 1. Section 3004 (u) of RCRA (Section 206 of the Hazardous <br /> and Solid Waste Amendments of 1984) and regulations <br /> promulgated as 40 C.F.R. 264. 101 and 270.32 (b) (2) , re- <br /> quire the Permittee to institute corrective action as <br /> necessay to protect human health and the environment <br /> 35 <br />
