RCRA Permit - Forward Landfill Is May 1991 <br /> EPA I.D. CAD 990 794 133 Modification 0 <br /> (4) Permit Modification <br /> Based on the information the Owner/Operator submits <br /> pursuant to Permit Conditions I .1 through X.3, EPA <br /> will select media protection standards for all hazard- <br /> ous waste and/or hazardous constituents released from <br /> SWMUs addressed under Permit Conditions IX.1 and 2. <br /> EPA also will specify which corrective measures the <br /> Owner / Operator shall implement to meet the proposed <br /> media protection standards as well as any conditions <br /> for submitting corrective action designs. EPA will <br /> propose these modifications as a major modification to <br /> this permit pursuant to 40 CFR Sections 124.5, 124.10 <br /> and 270.41. The Director will notify the <br /> Owner/Operator if specific corrective action measures <br /> shall be implemented under other EPA authority in ad- <br /> vance of permit modification in order to minimize en- <br /> vironmental releases. <br /> XI. ASSESSMENT OF NEWLY IDENTIFIED SOLID WASTE MANAGEMENT UNITS <br /> 1. The Permittee shall notify the Director of any newly <br /> identified SWMUs discovered during the course of ground <br /> water monitoring, field investigations, environmental <br /> audits or other means no later than fifteen (15) calen- <br /> dar days after discovery. <br /> 2. After such notification pursuant to permit condition <br /> XI. 1, the Director may request, in writing, that the <br /> Permittee prepare a Solid Waste Management Unit (SWMU) <br /> Assessment Plan and a proposed schedule of implementa- <br /> tion and completion of the Plan for any additional <br /> SWMU(s) discovered subsequent to the issuance of this <br /> Permit. The Assessment Plan shall contain the informa- <br /> tion listed in Attachment 6. <br /> 3. The Permittee shall notify the Director of any proposed <br /> waste management units within fifteen (15) calendar <br /> days after approval of design by local agencies and the <br /> Regional Water Quality Control Board. <br /> XII. OTHER PROVISIONS <br /> (1) The Owner/Operator shall report any imminent or exist- <br /> ing hazard to public health or the environment from the <br /> present or past release of hazardous constituents to <br /> the Director in accordance with Permit Condition VI.7. <br /> (2) If the Owner/Operator at any time determines that the <br /> RCRA Facility Investigation or Corrective Action Plan <br /> required under Permit Conditions IX and X no longer <br /> satisfy the requirements of Section 264. 101 or this <br /> 38 <br />
