CAD 990 794 133 Modification 0 <br /> (b) The RFI Report shall include an analysis and sum- <br /> mary of all facility investigations of solid waste <br /> management units and their results. The summary <br /> shall include a report on the type and extent of <br /> contamination at the facility, including sources <br /> and migration pathways, and description of actual <br /> or potential receptors. The report shall also <br /> describe the extent of contamination (qualitative/ <br /> quantitative) in relation to background levels <br /> indicative for the area. The objectives of this <br /> task shall be to ensure that the investigation <br /> data are sufficient in quality (e.g. , quality as- <br /> surance procedures have been followed) and quan- <br /> tity to describe the nature and extent of con- <br /> tamination, potential threat to human health <br /> and/or the environment, and to support a Correc- <br /> tive Action Plan, if necessary. The Owner / <br /> Operator shall provide sufficient written jus- <br /> tification for any omissions or deviations from <br /> the minimum requirements of Attachment 6. <br /> X. CORRECTIVE ACTION PLAN <br /> (1) The Director shall review the final reports on the RFI <br /> submitted under Permit Condition IX.2.a and notify the. <br /> Owner/Operator of the need for further investigative <br /> actions and/or the need for corrective action as re- <br /> quired under Section 264.101(a) . <br /> (2) If the Director determines that a solid waste manage- <br /> ment unit has had a release of hazardous waste or con- <br /> stituents which threatens or may threaten the environ- <br /> ment, the Owner/Operator shall develop a Corrective <br /> Action Plan for that unit and submit- it within 60 days <br /> after notification. The plan shall include activities ------- - <br /> necessary for removal and/or treatment of releases and <br /> any necessary monitoring of air, soil, and/or water to <br /> determine the adequacy of the actions. The plan <br /> shall also contain projected time schedules for im- <br /> plementation and completion of actions and for interim <br /> milestone activities. If the time necessary for im- <br /> plementation exceeds one year, the schedule shall <br /> specify interim dates for submittal of progress <br /> reports. <br /> (3) The Owner/Operator shall submit any Corrective Action <br /> Plan required by Permit Conditions IX.2 . to the Direc- <br /> tor for review and approval or modification. <br /> 37 <br />
