Harper, MHA, CHE <br />Senior Vice President/Area Manager <br />Central Valley Service Area <br />Please accept this correspondence as the Kaiser Permanente Stockton Medical Offices, 7373 West Lane <br />response to your findings and notice of violation on June 27, 2006. <br />During your routine inspection you made several findings to be addressed and corrected. The following <br />actions have been taken: <br />1. Violation, treatment/storage #16, HSC section 118215(2)- All medical waste shall be treated pursuant <br />to this section thereby rendering it solid waste. The biological indicator being used by the facility <br />expired in January 2006, waste was continuing to be treated from January 2006 through June 26, <br />2006 and being disposed as solid waste at a landfill. <br />Immediately, cease treating all medical waste at this facility until further notice by this office. <br />As of June 26, 2006 the Sanipac Sterilizer has not been utilized, subsequently all medical <br />waste has been hauled by Stericycle. <br />Immediately, remove all waste inside the sterilizer and all mixed waste from the compactor <br />and handle as medical waste. The mixed waste in the forty yard container was placed into <br />containment and hauled as medical waste by Stericycle. <br />Attached you will find a detailed report regarding the follow-up to these issues.
