Return to Compliance Timeline <br />Prepared By: Anna Mosher, Environmental Health and Safety Manger <br />July 6, 2006 <br />■ June 27, 2006 ceased use of the Sanipak, staff notified on all shifts, see <br />attachment. <br />■ June 27, 2006 engineering performed Lock Out Tag Out procedures on the <br />Sanipak/compactor waste bin, Anna Mosher authorized removal of lock on <br />6/28/06 for EVS to begin waste re -containerization. <br />® June 27, 2006 Solid waste pickups were canceled until all mixed waste was <br />removed. <br />■ June 28, 2006 waste management delivered a solid waste container for all solid <br />waste for use in the interim. <br />■ June 27, 2006 through June 30, 2006 mixed waste removed from the forty yard <br />bin, placed into containment, and hauled by Stericycle as medical waste. All <br />tracking documents are attached. <br />■ June 30, 2006 the forty yard bin previously containing the mixed waste was <br />decontaminated utilizing an approved chemical sanitizer listed in the medical <br />waste management act section 118295(b). <br />® June 30, 2006 upon decontamination of the bin, the compactor was once again <br />deployed for use and the solid waste bin was removed by Waste Management., <br />■ Replace expired bacteria spore ampules with new lot, and properly disposed of <br />expired lot. <br />Steps to ensure proper treatment procedures are followed: <br />■ Regain control of spore testing by removing Incubator from the Lab and placing <br />Incubator in the Environmental Services Manager's Office. <br />■ Sani-Pak Operator (Day Shift Waste Technician) will place bacteria spore <br />ampules (actual and control) in Incubator after actual ampule is autoclaved in the <br />first load, on the 20th of each month. <br />■ Waste Technician and the Environmental Services Manager or Environmental <br />Services Supervisor will verify and record results of test in the Attest Spore <br />Results Book <br />■ On the first Friday of each month, the Monthly Waste Management Check List <br />(see attachment) will be completed by Administrative Specialist, and verified by <br />the EVS Manger. <br />■ Any Discrepancies will be documented and reported to the Environmental Health <br />and Safety Manger and to the Administrator. <br />■ Re-train staff on polices and procedures regarding the Sanipak. See attached <br />training sign -in sheet and training topics. <br />■ Revised the Sanipak spore testing policy. <br />
