Harper, MHA, CHE <br />Senior Vice President/Area Manager <br />Central Valley Service Area <br />Mon E �C-�'E � V E 0 <br />� EB 2 0 2009 <br />ENVIRONMENTERMI/SERVICES HEALTH <br />Please accept this correspondence as the, Kaiser Permanente Stockton Medical Offices, 7373 West <br />Lane and Manteca Medical Offices 1721 West Yosemite Ave, response to your findings and notice of <br />violations on January 21, 2009. <br />During your routine inspection you made findings to be addressed and corrected. The violations are <br />outlined as follows: <br />Stockton Medical Offices: 7373 West Lane: <br />1. Violation, treatment/storage #15, HSC section 118765- Medical Waste treatment policy <br />varies from Policy book to policy book and have some variations. Organize policies so <br />they are consistent in every book. Spore test policy needs to specify the temperature at <br />which it is run. By 2/21/09 submit an updated spore test policy and medical waste <br />management plan. <br />2. Violation, treatment/storage #20, HSC section 118280- Medical Waste Labeling and <br />Containment. Medical biohazardous waste labels are lacking on (in autoclave area): 90 <br />gallon storage container and Red 32 gallon barrel with no lid and no label. <br />3. Violation, treatment/storage #21, HSC section 118285- Sharps labeling and <br />containment. Sharps containers need to be properly labeled with the words "sharps <br />waste" or the international biohazard symbol and the word "Biohazard". <br />4. There were also two notes (not violations) in addition to the violations as follows: A 32 <br />gallon red barrel was lined with an autoclave bag and a hazardous waste label. This can <br />cause confusion for employees so this waste was changed to a grey barrel for the <br />Hazardous waste storage. Spore tests showed one hour test instead of a three hours test <br />and this needs to be reviewed as the Rapid Attest 1292 requires three hour test. <br />Manteca Medical Offices: 1721 West Yosemite Ave: <br />1. Violation, treatment/storage #15, HSC section 118765- Medical Waste Tracking <br />Documents are missing for December 8, 2008 and January 12, 2009.
