Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: C00054169 Site Location: 21000 W HVVY 4 <br />Activity Summary <br />Activity Date Recorded by <br />08/20/2021 Called Coast Guard 707-374-2871 to provide an update on the food grade PRESTO <br />hydraulic oil spill. Informed them that the spill occured at the Middle River Pump <br />Station and went into the water way, the Victoria Canal. The spill is in the process <br />of being cleaned up. If there are any questions or additional information they need, <br />they can call our main line at 209-468-3420. <br />Called lead senior to provide update. <br />08/21/2021 9:00am-Left for 21000 Highway 4 to verify clean-up. BAKER <br />9:30am-Called Colton Rogers (925-899-6047) to check in and let him know I will <br />be there at 10 am. <br />10:00am-Arrived on site. Met with Colton Rogers (Environmental Health & Safety <br />Specialist). Clean-up is complete. No sheen or oil observed on water surface. <br />Concrete is clean. Eight 55 gallon drums of waste was generated from the <br />clean-up. Waste was labeled as hazardous waste and will disposed of this week <br />after a temporary ID number is obtained. Left site at 10:16 am. <br />Completed photo log. <br />9:00am-11:52 am. <br />08/23/2021 Received update and photo log from Lydia Baker. Cleanup has been completed. ALANIZ <br />08/23/2021 Completed: ALANIZ <br />-Filling out complaint <br />-Prop 65 <br />-8/20/2021 narrative <br />-8/20/2021 photo log <br />08/25/2021 A voicemail was left on the backup ER phone on 8/23/2021 from Warden Schutz PRESTO <br />(209-642-0465). He is writing an after hours report and has a couple of questions. <br />Called him back today and left a voicemail informing him the number he called is <br />not a routinely monitored phone and provided him my phone number if he still had <br />questions. Also informed him in the voicemail that the clean up was completed on <br />8/21/2021, no sheen or oil was observed on the surface water. <br />5105 rpt Page 2 of 2
