If the address <br />of Lodi Memorial Health Corporation (975 S. Fairmont) or the phone number (209-334-3411) has changed because of <br />this new affiliation, please let me know and I will update our records. Our major concerns are facility ownership changes <br />and changes to staff that we contact for our inspections. From reading your letter, I understand that Lodi Memorial <br />Hospital Association is still the owner of the facility and that the facility will operate in the same manner with the same <br />staff as it did before. <br />If you have any questions or concerns, please email me and I will respond as soon as possible.
