OBSERVATION: An approved copy of the monitoring plans were not available for inspection. REGULATION GUIDANCE: A copy of these plans shall be accessible on site at all times. CORRECTIVE ACTION: Immediately locate and retain a copy on site. This is a repeat violation, Class II. 208 CCR 2716(a -e) Failure to comply with the designated operator inspection requirements. 1. OBSERVATION: The designated operator failed to check and note the correct test dates for the Spill Containment Test on the designated operator inspection reports. The correct date for Spill Containment Test is 12/12/2022 but was listed as 12/22/2022 on multiple designated operator reports. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. 2. OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: - Multiple Designated Operator Reports did not have Section 5 completed for owner to respond to Designated Operator Compliance Issues listed in Section 3 - The 11/29/2023 Designated Operator Report did not have Section 7 and 8 completed and did not have Mechanical float mechanisms used in UDCs checked in Section 9 - The 11/02/2023 Designated Operator Report noted that there was liquid in UDC 9/10 in Section 3 but marked that UDC 9/10 was free of damage, water, debris and hazardous substances in Section 9 REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II.
