Fairmont Ave., <br />Lodi, CA 95240 and Lodi Memorial Hospital West at 800 S. Lower Sacramento Rd., Lodi, CA. <br />Lodi Memorial Hospital <br />1. Section 118310 <br />a. Correction on the Biohazardous signage required by Section 118310 "CAUTION— <br />BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED PERSONS KEEP <br />OUT," and in Spanish, "CUIDADO—ZONA DE RESIDUOS—BIOLOGICOS <br />PELIGROSOS—PROHIBIDA LA ENTRADA A PERSONAS NO AUTORIZADAS," <br />has been ordered and will be in place by 6-20-10. <br />2. Section 118275 (a.) <br />a. A bag of blood/plasma was found in room E2H535 still attached to an Ortho Pat <br />machine was removed and appropriately place in a biohazardous waste storage <br />container the same day as the inspection. Staff was appropriately instructed on the <br />proper removal and disposal of this type of waste. Complete 5-20-10. <br />3. Section 118280 <br />a. The biohazard symbol or word "Biohazard" was not found on numerous containers <br />for the biohazard waste. The vendor, Stericycle has been notified and will provide <br />compliance by 6-20-10. <br />4. Section 118275 (d) <br />a. A chemotherapy waste container lacked the sign "Chemotherapy Waste" or <br />"CHEMO" on all sides of the container and lid. Labels were purchased and have <br />been appropriately applied to the containers. Complete 5-27-10. <br />
