Please review the inspection report. #111 discusses overfill <br /> test results dated 8/26/2022 that were submitted to EHD. #111 does not discuss anything regarding fuel drop requests. <br /> In addition you state that no testing was conducted that day, 8/26/2022,yet EHD received test results for 8/26/2022 for <br /> overfill testing.These overfill test results for 8/26/2022 were also included in the previously submitted return to <br /> compliance packet. EHD was not notified of any overfill testing that was scheduled for 8/26/2022. We were notified of <br /> the 8/30/2022 overfill testing and were on site.The email correspondence from your previous packet shows a request to <br /> schedule testing 8/30/2022 -8/31/2022 (which included overfill) and ELD on 8/25/2022. Review details in the inspection <br /> report amended 12/29/2022 and corrective actions required. <br /> #115—closed based on statement <br /> #203— <br /> 1. The attached is not an alarm history and if the panel was replaced same day 6/6/2022 then you should have had <br /> a working panel 6/6/2022 through project completion.The document submitted shows a time period before the <br /> panel was replaced. We need it after it was replaced throughout the duration of the installation and upgrade <br /> construction project. <br /> 2. See the inspection appointments on the temporary closure permit.The tanks did not go through any of the <br /> inspections listed on the temporary closure.The temporary closure permit was approved in case Costco decided <br /> to temporarily close the tanks during the project so Costco already had a permit in place but the tanks were not <br /> temporarily closed.To be temporarily closed the product needs to be removed and product was kept in the <br /> tanks. <br /> 3. The attached work order with the Order Date of 9/30/2022 does not reference at all a technician coming out <br /> and determining the drop tube to be faulty. It only references that drop tubes were installed 10/10/2022 on the <br /> Tank 2 87B tank. Bruno Espinoza's email from 11/4/2022 stated that a technician came out on 9/27/2022 to <br /> inspect the tubes and determined faulty. Do technicians typically not leave work orders on site when they do <br /> inspections on your system?We typically see work orders on site at facilities even for inspecting a UST <br /> component and not doing any work that day. How was the site notified of the faulty drop tube? How are you <br /> keeping records of issues with the UST system if technicians don't leave work orders? <br /> 4. Was there an overall end of project work order submitted for the tank installation and upgrade completion? <br /> • For the request for the work order for 9/14/2022, 1 noticed what was sent 1/26/2022 was actually a work order <br /> with a trip date of 9/13/2022.Was Bruno's email a typo on the date stating that a technician came out and <br /> verified drop tubes were correct measurements on 9/14/2022 and he actually meant to say 9/13/2022? <br /> 1 <br />
