According to the contractor <br /> there was no testing completed or CUPA onsite on that day to due to scheduling conflicts and availability of CUPA <br /> inspector.Jones Covey requested information from CUPA in regards to remaining requirements to drop fuel on 8-26-22. <br /> They were unable to maintain that schedule, and as such, scheduling for CUPA on 8-30 and 8-31 was requested to <br /> finalize the required inspections in order to receive fuel. CUPA was onsite 8-24 for final ELD testing verification. <br /> Item#115—The facility will ensure to have complete alarm histories showing all tested sensors will be provided moving <br /> forward. <br /> Item#203 - <br /> 1. Alarm history print out for all alarms that occurred after June 2022 designated operator report until the <br /> monitoring panel replacement that occurred during the 91 tank installation and fueling station upgrade.The <br /> alarm history provided was for 5/1/2022 to 6/7/2022.The history needed would be after the time frame you <br /> provided.The alarm history need would be from whenever the June 2022 designated operator report was <br /> through when the panel was replaced. For reference the tanks were placed in the ground on 6/22/2022 and all <br /> of the final testing occurred 8/30/2022. <br /> a. The panel was replaced in the same day, which was completed on June 6, 2022 prior to the next DO <br /> Inspection.The data attached is from the time period of the closest inspection to the day after <br /> replacement. <br /> 2. Alarm logs and response records for all alarms that occurred during the 91 tank installation and fueling station <br /> upgrade.This means responses to each alarm that occurred and that they were responded to as the tank was <br /> not temporarily closed during the renovations, so the UST should have been continued to be monitored during <br /> this whole renovation. <br /> a. Please see the attached Permit issued for the temporary closure of this tank, issued on May 17, 2022. <br /> Please verify this violation is no longer valid as the tank was permitted for Temporary Closure. <br /> 3. Jones Covey work order for 9/27/2022 for drop tube inspection was not attached. Per Bruno's email on page <br /> 144 of your document, he stated they received a call on 9/27 and a technician inspected the drop tubes.A work <br /> order was not attached. Emails were attached from Jones Covey on page 193 of your document that T2 was <br /> reported faulty but a work order was not attached for the determination that it was faulty. <br /> 2 <br />
