Alarm history print out for all alarms that occurred after June 2022 designated operator report until the
monitoring panel replacement that occurred during the 91 tank installation and fueling station upgrade. The
alarm history provided was for 5/1/2022 to 6/7/2022.The history needed would be after the time frame you
provided. The alarm history need would be from whenever the June 2022 designated operator report was
through when the panel was replaced. For reference the tanks were placed in the ground on 6/22/2022 and all
of the final testing occurred 8/30/2022.
a. In the future, the CCR 2712(b)(1-2) requirements will be met.
2. Alarm logs and response records for all alarms that occurred during the 91 tank installation and fueling station
upgrade. This means responses to each alarm that occurred and that they were responded to as the tank was
not temporarily closed during the renovations, so the UST should have been continued to be monitored during
this whole renovation.
a. Please see the attached alarm logs from the facility for alarms that occurred during the 91 tank
installation and fueling station upgrade with responses provided by Jones Covey Group. In the future,
the CCR 2712(b)(1-2) requirements will be met.
i. Alarms from 8/31: Generated during system startup and calibration
ii. Alarms from 8/30: Generated during the Monitoring Certification
iii. Alarms from 8/29: Generated during the Monitoring Certification pre-testing
iv. Alarms from 8/28: Generated during Monitoring Certification pre-testing
v. Alarms from 8/27: Generated during continued dry-monitoring certification
3. The attached work order with the Order Date of 9/30/2022 does not reference at all a technician coming out
and determining the drop tube to be faulty. It only references that drop tubes were installed 10/10/2022 on the
Tank 2 87B tank. Bruno Espinoza's email from 11/4/2022 stated that a technician came out on 9/27/2022 to
inspect the tubes and determined faulty. Do technicians typically not leave work orders on site when they do
inspections on your system? We typically see work orders on site at facilities even for inspecting a UST
component and not doing any work that day. How was the site notified of the faulty drop tube? How are you
keeping records of issues with the UST system if technicians don't leave work orders?
a. Attached are emails and work orders from Jones Covey Group. Please confirm if you need any additional
information to resolve this violation.
i
