The plan provides processes for establishing an alternate care site(s) within <br /> the community that has the capabilities to meet the needs of patients when <br /> the environment cannot support adequate care, treatment, and services <br /> including processes for the following: <br /> 1 Transporting patients, staff, and equipment to the alternative care <br /> site(s) <br /> 2. Transferring to and from the alternative care site(s), the necessities of <br /> patients (for example, medications, medical records) <br /> 3. Tracking of patients <br /> 4. Inter-facility communication between the hospital and the alternative <br /> care site(s) <br /> M. The plan provides processes for identifying care providers and other <br /> personnel during emergencies. <br /> N. The plan provides processes for cooperative planning with health care <br /> organizations that together provide services to a contiguous geographic area <br /> (for example, among organizations serving a town or borough) to facilitate the <br /> timely sharing of information out the following: <br /> 1 Essential elements of their command structures and control centers for <br /> emergency response <br /> 2. Names and roles of individuals in their command structures and <br /> 40 command center telephone numbers <br /> 3. Resources and assets that could potentially be shared in an emergency <br /> response <br /> 4. Names of patients and deceased individuals brought to their <br /> organizations to facilitate identifying and locating victims of the <br /> emergency. <br /> O. The plan identifies under what circumstances and the process for <br /> communicating patient information to relevant third parties, such as FBI and <br /> Health Department. <br /> P. The plan identifies back-up internal and external communication systems in <br /> the event of failure during emergencies. <br /> Q. The plan identifies alternate roles and responsibilities of staff during <br /> emergencies, including to who they report in the hospital's command <br /> structure and, when activated, in the community's command structure. <br /> R. The plan identifies an alternative means of meeting essential building utility <br /> needs when the hospital is designated by its Emergency Operations Plan to <br /> provide continuous service during an emergency (for example, electricity, <br /> water, ventilation, fuel sources, medical gas/vacuum systems) for up to 960. <br /> S. The plan provides alternatives in the event the hospital cannot sustain <br /> services for any period of time (for example evacuation). <br /> T.
