Pharmaceutical waste will be segregated in containers at specific locations near medication <br />preparation areas. <br />d. Pharmaceutical waste container holders will be wiped clean at least once a week using an EPA <br />approved disinfectant. <br />e. Pharmaceutical waste containers will not be placed inside a red bag or in regular trash. <br />f. Once placed into the container, pharmaceutical waste will not be removed for any reason. <br />5. Controlled Drug Disposal: <br />Controlled drugs will be disposed of in compliance with Federal Drug Enforcement Administration (DEA) <br />regulations. <br />a. All intact unit -dose and partial bulk containers of outdated (or beyond use date) Schedule II, III, IV, <br />and V controlled drugs will be disposal by a DEA -registered pharmaceutical returns <br />company(currently Guaranteed Returns Rx). <br />a. Arrangements will be through Materials Management for pick up by Guaranteed Returns Rx for <br />pick up and disposal of the controlled substances that are intact (still in unopened package or <br />vial). <br />b. All disposals (wasting) of individual controlled substance doses will be recorded and witnessed by <br />two healthcare personnel according to policies. <br />c. Syringes, ampules, vials; patches, tablets, capsules, lozenges, suppositories,,and bags, bottles, or <br />cassettes for IV infusion containing partial doses of: controlled drugs will be discarded directly into the <br />pharmaceutical waste container immediately following documentation and witnessing of waste. <br />1. 'Devices containing liquid controlled, substances it may be emptied or squirted into the <br />pharmaceutical waste container if it is less than 15 ml before being placed into; pharmaceutical <br />waste. If it is wasted it must be witness and signed by two staff members ie. RN, LVN, MA or a <br />physician. <br />2. Full doses that have been refused by the patient or partial doses of liquid controlled substances <br />in open containers (dose cups) may be discarded by pouring into the pharmaceutical waste <br />before the waste. If the volume to be wasted is greater than 15 mL, the liquid will be placed into <br />a LARGE (8 gal.) pharmaceutical waste container. <br />6. Medications Brought To The Office By Patients For Disposal: <br />a. Except in circumstances requested by the physician we cannot accept patient medications for <br />disposal <br />1. Instruct patient on proper disposal of medications (see Appendix B) <br />2. In situations where the provider requires the patient to return the medication before prescribing <br />a new medication: <br />a. The Medical Assistant or nurse is to document in the patient record "per [provider's name], <br />the patient returned [drug name, strength and quantity].The medication has properly been <br />disposed of in the Pharmaceutical Waste Container and witnessed by [co-worker's name]". <br />b. Two people must witness this receipt and disposal of controlled meds. <br />c. Pills should be dumped into the Pharmaceutical waste container and the bottle with any <br />Pharmaceutical Waste.
