OBSERVATION: The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 5/8/2022, next inspection performed on 6/8/2022 (31 days) - Initial inspection on 7/7/2022, next inspection performed on 8/7/2022 (31 days) - Initial inspection on 12/4/2022, next inspection performed on 1/5/2023(32 days) REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. ---------------------------------------------------------- OBSERVATION: The designated operator failed to check and note the correct test dates for the overfill prevention inspection on the designated operator inspection reports. The overfill prevention inspection date listed was 3/25/2019. The correct date for the overfill prevention inspection is 4/15/2022. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 302 CCR 2630(d), 2641(a)Leak detection equipment not installed/maintained/located to detect leaks at earliest opportunity. OBSERVATION: The 87-grade sensor was located on the top of the saddle tank and not located to detect a leak at the earliest opportunity. REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. CORRECTIVE ACTION: This was corrected on site. The sensor was relocated to the lowest point of the sump by the service technician. Ensure that all monitoring equipment is maintained and located to detect a leak at the earliest opportunity.
