Complaint Investigation Summary Report#i 5105 COMPLAINT ID: COK24HKHW Site Location: 1-5 NORTHBOUND, JUST SOUTH OF W PELTIER RD, LODI, CENTER DI Activity Summary Activity Date Recorded by 05/03/2022 Arrived on site and met with Sargent Curtis Glace with CHP and Loren Shelton, PRESTO Maintenance Supervisor with CalTrans. Per Sergent Glace the driver fell asleep gong southbound on 1-5, hit the guard rail and caused the truck to flip on its side causing the release. The driver only sustained minor laceration and went home. On EHD arrival, the truck was already turned right side up and was in the process of being hauled away. The truck and associated accident debris was hauled by towing company, Lodi Heavy and Haul. The clean-up company on site was Ancon Environmental Services. Loren pointed out impacted areas from the diesel spill. The diesel released on the asphalt, dirt in the shoulder, and dirt under the center-divide guard rail. After the accident, traffic was redirected through the center divide and spread some of the impacted dirt a little further down. Received the responsible party information from Loren - ProPacific Fresh. Ancon Services cleaned the diesel spill off of the asphalt with absorbent sand. No waste will be hauled off today. The absorbent was placed on top of the impacted dirt in the center-divide shoulder to absorb diesel release on the dirt. The absorbent sand was covered with visqueen plastic and surrounded with cones. Absorbent sand bags were placed throughout the top of the visqueen plastic to keep it in place. Clean up to be completed another day after USA dig. (continued from 5/2/2022 daily. Total time for complaint: 5/2/2022 9:30pm - 5/3/2022 1:40am) 05/03/2022 Created complaint. Started notice to abate. PRESTO 05/04/2022 Completed notice to abate and emailed to Kim Bresson, Pro Pacific Fresh PRESTO Transportation Supervisor (kbresson@propacificfresh.com). Created Prop65 and placed in mailing bin. Created emergency response record, photo log. Printed 5104, photo log, notice to abate and emailed for packet. 05/09/2022 Received spill report update request from Dong, Weijie@Waterboards MANZO <Weijie.Dong@Waterboards.ca.gov>, provided a summary email and a narrative for this complaint to Dong, Weijie@Waterboards <Weijie.Dong@Waterboards.ca.gov> via email. 05/23/2022 Received notice from Juan Armendarez, clean-up scheduled with Ancon for PRESTO 5/24/2022 at 9:OOam. 05/24/2022 Arrived on site to verify clean up. Met with Juan Ochoa with Ancon and Juan PRESTO Armendarez with Caltrans. Clean up is still in process but almost all of the excavated soil has been excavated. The excavated soil will be taken to Caltrans yard on Aurora and Stanislaus while the soil is pending analytical. The soil was placed in two 20 cubic yard roll offs and is waiting for a third 20 cubic yard bin for the rest of the soil. Left site and Juan Armendarez will send photos of completed excavation and backfill. Received photos of completed excavation and backfill with the final excavated amount at 60 cubic yards to be taken to crosstown storage. Ancon to submit disposal records and analysis results.
