Sterilization certificates were not available for Kwadron needle lot# 23083 and Cheyenne needle cartridge lot #2024-39- <br />142 <br />CORRECTIVE ACTION: <br />1. A log of all procedures, including the names of the practitioner and client and the date of the procedure must be <br />maintained when using disposable, single-use, pre-sterilized instruments. Moving forward, ensure to include all required <br />information on client records. <br />2. Provide sterilization certificates for Kwadron needle lot# 23083 and Cheyenne needle cartridge lot #2024-39-142 <br /> <br />#13. HSC 119307 - Bloodborne Pathogen training <br />OBSERVATION: The bloodborne pathogens training certificate for Saul was not issued by an approved provider. <br />CORRECTIVE ACTION: Submit a bloodborne pathogens training certificate issued by one of the approved providers on the San <br />Joaquin County’s list. <br /> <br />#18. HSC 119303 - Client records approved and available - Consent form and questionnaire <br />OBSERVATION: The 6/4/2025 consent form for Andrew Nathaniel Boyd was not signed by the client. <br />CORRECTIVE ACTION: Prior to the performance of body art, the client shall read, complete, and sign an informed consent <br />form. Moving forward, ensure all consent forms are signed by the client. <br /> <br />#19. HSC 119303, 119308 - Appropriate aftercare instructions given to client <br />OBSERVATION: The after-care instructions do not include signs and symptoms of infection and indications when to seek <br />medical care. <br />CORRECTIVE ACTION: Submit updated after-care instructions that include all required information. <br /> <br />#35. HSC 119313 - Operation and employee training records present <br />OBSERVATION: The facility could not provide training records for the Infection Prevention Control Plan (IPCP). <br />CORRECTIVE ACTION: IPCP training must be provided when tasks are initially assigned, any time there is a change, when <br />new technology is adopted for use, but not less than once a year. Provide IPCP training to all employees and submit a copy of <br />the training record. <br /> <br />#37. HSC 119312 - Permits obtained and available <br />OBSERVATION: Saul Esparza does not have a current practitioner permit. <br />CORRECTIVE ACTION: Provide proof of permit fee payment. <br /> <br /> <br /> <br /> <br />
