Emailed inspection report to facility owner, Paul Marquez, at pmarqueztattoos209@gmail.com. <br />2. Reviewed the Infection Prevention Control Plan (IPCP), consent forms, medical questionnaire, and after-care <br />instructions. <br />3. Reviewed disposal records. The facility is utilizing Rynocare for their sharps disposal. Ensure to maintain records for <br />3yrs. <br />4. Per Paul, practitioner Isaac Gonzales has not started working at the facility yet. He anticipates Isaac to start soon. <br />Ensure all fees are paid before beginning work. <br />5. We received returned mail for practitioner Isaac Gonzales’ invoice. I provided Paul with a copy of Isaac’s invoice during <br />the inspection and requested that Isaac contact me to update his mailing address. <br />6. The procedure chair in Isaac’s station is torn, but it is not currently in use. Ensure that the chair is replaced or repaired <br />(patching not accepted) prior to Isaac beginning work. <br />7. Per Paul, he is unsure if Byron will be continuing to provide body art services due to medical reasons. <br />8. Paul provided confirmation of facility and practitioner permit fee payment for 2025 fees. <br />9. Per Paul, his son will start helping at the facility and has already taken his bloodborne pathogens training. He will receive <br />some training from Paul but it will not be hands-on. I instructed Paul to ensure his son gets registered as a practitioner <br />through our department before performing any body art. <br /> <br /> <br />
