OBSERVATION: The designated operator failed to check and note the correct test dates for the monitoring system certification and overfill prevention on the designated operator inspection reports. The monitoring system certification test date was listed as 4/7/21 and 4/29/22.The overfill prevention date was listed as 11/22/19.The correct date for the monitoring system certification is 4/28/22 and the correct date for the overfill prevention is 10/11/21. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. OBSERVATION: The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 1/12/23, next inspection performed on 3/8/23 (55 days) REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. OBSERVATION: Designated operator reports were not signed within the required time frame. -The 1/12/23 designated operator report was not signed by the owner/operator. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This is a repeat violation, Class II.
