Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00054426 Site Location: 29050 E WIMER RD <br />Activity Summary <br />Activity Date Recorded by <br />12/02/2021 Received PCB Sample results from John Alaniz, REHS, Reviewed sample results <br />from Castle Analytical Laboratory. Thirty samples was tested for Polychlorinated <br />SAETERN <br />Biphenyls (PCB) (mg/kg). The following samples came back positive: <br />- C2 (2111122-12) Soil: 0.072 mg/kg <br />- D2 (2111122-17) Soil: 0.13 mg/kg <br />- D3 (2111122-18) Soil: 0.028 mg/kg <br />- D4 (2111122-19) Soil: 0.018 mg/kg <br />- E2 (2111122-21) Soil: 0.028 mg/kg <br />- DUP1 (2111122-27) Soil: 0.19 mg/kg <br />12/03/2021 Witnessed sampling of C2 and D2 for further ALANIZ <br />PCB analysis. Per Alex, Dave Larsen conducted further excavation of the two <br />sampling points on 12/1/2021. Approximately 20 pounds of oily debris was <br />generated and taken to 4040 West Ln, Stockton. <br />Created a photo log and filed photos. <br />12/03/2021 Updated Kristina Saetern, EHS, and Elena Manzo, lead senior. ALANIZ <br />12/09/2021 Called Alex Steele for the PCB limit definition and PCB results. Alex told me the ALANIZ <br />CFR code and said that the PCB results came back as non-detect. He said he <br />would email the results. <br />12/09/2021 John Alaniz updated with PG&E responsed with CFR Code for PCB. SAETERN <br />01/10/2022 Discussed compliant with Kristina Saetern, EHS. ALANIZ <br />01/10/2022 Compiling files for review. SAETERN <br />Send e-mail to Alex Steele, requesting for second test results and disposal <br />records. <br />01/11/2022 Generated Emergency Response Records, photo logs, SAETERN <br />01/27/2022 Phoned Alex Steele of PG&E, 209-337-8902, left a message regarding a manifest. SAETERN <br />02/02/2022 Completed er record. ALANIZ <br />02/02/2022 Per request of Gudino, Randy@Waterboards MANZO <br /><Randy.Gudino@Waterboards.ca.gov > for the complaint record checked on the <br />complaint package status with K. Saetern, scanned and organized complaint <br />record, downsized PDF file (too large for the email) and emailed to Gudino, <br />Randy@Waterboards <Randy.Gudino@Waterboards.ca.gov> with cc. to M. <br />Nissim, program coordinator. <br />02/10/2022 Ennailed Alex Steele, requesting he submit the final disposal record.
