Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: COTGGKZ9Z Site Location: NB 1-5 RAMP TOWARDS EB ST RTE 120 <br />Activity Date <br />Activity Summary <br />Recorded by <br />04/27/2021 7:30 En route to diesel spill. MURO <br />8:03 AM Arrived to diesel spill site. Observed that the area that was damned to <br />have been damned with existing soil. Took photos of damned soil and <br />visqueen-covered diesel spill. The visqueen was weighted down with new sealed <br />bags of absorbent. Observed a trail of diesel about 6 inches wide and about 20 <br />feet in distance that was not covered up as it was a minimal amount of diesel <br />drizzled onto the road shoulder. <br />Discussed with Lead Senior Garrett Backus (EHD). <br />Left spill site. <br />8:35 AM Arrived at EHD. <br />Generated complaint. <br />Called Jose Diaz, (Diaz Transport) 831-970-0959. I spoke to him in Spanish, <br />since he had difficulty understanding me. I explained I would be emailing him the <br />Notice to Abate for cleanup and proper disposal of the wastes generated from the <br />diesel spill I let him know I was aware that Ancon Services was on scene and had <br />begun taking steps for the cleanup process. He provided his email address. <br />Poncho2000@live.com <br />Called Norman Sanchez, (Ancon Services), 916-581-9982. Norman reported <br />cleanup will begin next week, and will advise once it has been scheduled. He <br />reported Juan Armendarez was also present at the scene. <br />The spill site is difficult to find, since there is no address. In the event I will not be <br />available for the cleanup verification, I generated coordinates for the spill location. <br />37°4720.3"n 121°1716.0"w <br />Completed Notice to Abate. <br />04/28/2021 Emailed Notice to Abate to Jose Diaz (Diaz Transport) at: poncho2000@live.com. MURO <br />Completed Emergency Response Record, Site Map, Prop 65. <br />Emailed Prop 65 to Lead Seniors Michelle Henry, Stacy Rivera, Haza Saeed for <br />review (wasn't sure who was available so 1 emailed to all three). <br />04/29/2021 Emailed Prop 65 to Lead Senior Garrett Backus, since it was not reviewed <br />yesterday. <br />MURO <br />04/29/2021 Completed Photo log MURO <br />05/03/2021 Called Norman Sanchez (Ancon Services) 916-581-9982, for cleanup scheduling <br />information. Left message. Norman called back and left voice message that <br />cleanup had to be pushed back a week, did not report why. Cleanup will be <br />scheduled between May 12- 14. <br />MURO <br />05/03/2021 Discussed spill with Lead Senior Elena Manzo (EHD). MURO <br />05/10/2021 Follow-up phone call to Norman Sanchez (Ancon). Cleanup will commence on MURO <br />Thursday, May 13 at 9:00 AM.
