Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COTGGKZ9Z Site Location: NB 1-5 RAMP TOWARDS EB ST RTE 120 <br />Activity Summary <br />Activity Date Recorded by <br />05/11/2021 Per Juan Armendarez (CALTRANS), cleanup will be on Friday, 5-14. Called MURO <br />Norman Sanchez to confirm. Norman Sanchez reported that cleanup will be on <br />5/14. <br />05/12/2021 Provided complaint information to Senior REHS John Alaniz (EHD) since I am <br />unavailable on the day of cleanup and he will be on-call for emergency response <br />on 5/14 and will be verifying cleanup. Emailed John a PDF of complaint <br />documents compiled so far. Emailed Juan Armendarez (CALTRANS) to notify that <br />John will be the one to call for the spill cleanup. Juan reported that Loren Shelton <br />will be overseeing the cleanup on Friday. The cleanup is scheduled to start at 9:00 <br />AM and asks that whoever is going to the spill site, to plan accordingly as it is not <br />Caltrans's responsibility to call the EHD, unless there is a problem or a change in <br />plans. <br />MURO <br />Loren Shelton's phone number is: 209-483-4466. <br />I let Juan know I will make note of that for future cleanups. <br />05/12/2021 Discussed complaint with Claudia Muro, REHS. Will be responding to cleanup for ALANIZ <br />her. <br />05/17/2021 Discussed spill with Claudia Muro. Will not need to respond to cleanup. ALANIZ <br />05/17/2021 Notified John Alaniz that he is no longer going to be verifying the cleanup for this MURO <br />spill as my availability has opened. <br />05/18/2021 Called Nolan Ryan (Ancon Services) at 916-701-3376 for cleanup status He MURO <br />reported cleanup will be today. The site foreman will be Alejandro Samano <br />(510-292-8099). Called Alejandro Samano and requested they call me when near <br />completion of cleanup. <br />9:20 AM Left EHD. <br />9:42 AM Arrived to spill site. Met with Juan Armendarez (CALTRANS) and <br />Alejandro Samano (Ancon). <br />Excavation Details: <br />An area of approximately 10 x 20 feet was excavated to depth ranging from 8 - 20 <br />inches. No oily staining was observed. No odor was detected. The trail of diesel <br />that was evident during my site visit on 4-27 was no longer visible. Site was <br />observed to have been adequately cleaned. <br />One 20 yard bin was partially filled more than halfway with the diesel-contaminated <br />soil. The waste is pending analytical and will be stored at the CALTRANS yard <br />located adjacent to the 1-5 and Dell Osso Family Farms. <br />06/17/2021 Read letter and attached CHP Traffic Crash Report received June 14, 2021 from <br />National Claims Management. National Claims management is disputing liability of <br />the accident because the CHP has named the other party as the responsible party <br />for making an unsafe turn.
