MONITORING ARID REPORTING PROGRAM 13— <br /> FORWARD INCORPORATED <br /> WASTE MANAGEMENT FACILITY <br /> CLASS 11 LANDFILLS <br /> CLASS III LANDFILLS <br /> CLASS II SURFACE IMPOUNDMENT <br /> CLASS II LAND TREATMENT UNIT <br /> SAN JOAQUIN COUNTY <br /> MONITORING AND REPORTING PROGRAM REVISION <br /> This monitoring and reporting program will be revised by the Board under any <br /> of the following conditions: <br /> A. Expansion of WMU "D" or "E". <br /> B. Construction of WMU "F"°. <br /> C. Movement of WMU "G". <br /> D. Construction of WMU "N". <br /> REPORTING <br /> In reporting the monitoring data, the Discharger shall arrange the data in <br /> tabular form so that the date, the constituents, the concentrations, and the <br /> units are readily discernible. The data shall be summarized in such a manner so <br /> as to illustrate clearly the compliance with waste discharge requirements. <br /> Monthly monitoring reports shall be submitted to the Board by the 15th day of <br /> the month following the month in which the samples were taken. Quarterly, <br /> semiannual, and yearly monitoring reports shall be submitted to the Board by the <br /> 15th day of the month following the calendar quarter in which the samples were <br /> taken. <br /> The results of any monitoring done more frequently than required at the <br /> locations specified herein shall be reported to the Board. <br /> A report shall be submitted to the Board by 30 January of each year containing <br /> both tabular and graphical summaries of the monitoring data obtained during the <br /> previous year. The report shall include a discussion of compliance with the <br /> waste discharge requirements. <br /> The Discharger shall implement the above monitoring program on the effective <br /> date of this Order. <br /> Ordered by <br /> ILLIAM H.
