MONITORING AND REPORTING PROGRAM <br />FORWARD INCORPORATED <br />WASTE MANAGEMENT FACILITY <br />CLASS II LANDFILLS <br />CLASS III LANDFILLS <br />CLASS 11 SURFACE IMPOUNDMENT <br />CLASS 11 LAND TREATMENT UNIT <br />SAN JOAQUIN COUNTY <br />-13- <br />NONITORING ANII REPORTING PROGRA14 REVISION <br />This monitoring and reporting program will be revised by the Board under any <br />of the following conditions: <br />A. Expansion of WMU "D" or "E". <br />B. Construction of WMU "F". <br />C. Movement of WMU "G". <br />D. Construction of WMU "H". <br />In reporting the monitoring data, the Discharger shall arrange the data in <br />tabular form so that the date, the constituents, the concentrations, and the <br />units are readily discernible. The data shall be summarized in such a manner so <br />as to illustrate clearly the compliance with waste discharge requirements. <br />Monthly monitoring reports shall be submitted to the Board by the 15th day of <br />the month following the month in which the samples were taken. Quarterly, <br />semiannual, and yearly monitoring reports shall be submitted to the Board by the <br />15th day of the month following the calendar quarter in which the samples were <br />taken. <br />The results of -any monitoring done more frequently than required at the <br />locations specified herein shall be reported to the Board. <br />A report shall be submitted to the Board by 30 January of each year containing <br />both tabular and graphical summaries of the monitoring data obtained during the <br />previous year. The report shall include a discussion of compliance with the <br />waste discharge requirements. <br />The Discharger shall implement the above monitoring program on the effective <br />date of this Order. <br />Ordered by kA <br />WILLIAM H.
