The following conditions for soil classification, <br /> acceptance, treatability, and final disposal will apply: <br /> Classification <br /> 1. Forward, Inc. may receive only petroleum hydrocarbon <br /> contaminated soils or metal contaminated soils with metal <br /> concentrations and types as contained in the RWQCB Waste <br /> Discharge Requirements Order No. 89-148 . Petroleum products <br /> acceptable include gasoline, diesel, jet fuel, kerosene, non- <br /> hazardous oils and other petroleum distillates. <br /> 2 . Contaminated soils that have a DOHS variance may be accepted. <br /> Acceptance <br /> 1 . Contaminated soils accepted for treatment and/or disposal at <br /> Forward Inc. must be non-hazardous according to CCR Title 22 , <br /> Article 11�'. Hazardous wastes are not acceptable. <br /> 2 . Soils containing WDR-specified designated levels of metals or <br /> petroleum hydrocarbons may be accepted by Forward. <br /> •3 . The following completed package documents are required of all <br /> applicants proposing to dispose of contaminated soils at <br /> Forward: <br /> (a) Contaminated soil description form <br /> (b) waste characterization form <br /> (c) Test-analysis form <br /> (d) Chain of custody <br /> 4 .
