Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COGENUJ36 Site Location: IN FRONT OF 1049 S WILSON WAY <br />Activity Summary <br />Activity Date Recorded by <br />08/28/2021 9:03 am, called Jocelyn 415-710-0074 with Arcadis, no answer. Called Zantha MANZO <br />925-597-7321 with PG&E, left a voice message asking for a call back with today's <br />schedule. Called Dave Larsen 209-401-9719 (PG&E), Dave provided an update, <br />they are removing concrete and excavating soil and Arcadis doing confirmation <br />soil sampling. 10:29 am, received a call back from Zantha, went over the plan for <br />confirmation soil sampling. Reviewed a sampling map sent by Zantha. (12 min) <br />11:18 am left to go to the site. 11:54 am, arrived onsite, PG&E, Arcadis, PSC and <br />Clean Harbor crews are onsite working on concrete removal. Jocelyn with Arcadis <br />collected one concrete grab sample from inside the storm drain inlet wall (stand <br />pipe) just below the top hat, some darker soil was noted in a small area (2") <br />between the storm drain top hat and the concrete stand pipe and pointed out to <br />Jocelyn with Arcadis. Concrete was removed from the gutter area, pavement was <br />removed from the driveway area, additional soil excavation was conducted around <br />the power pole in the area of two rain gutter drain pipes that exited from below the <br />sidewalk at street level next to the power pole. Arcadis collected soil samples from <br />below the removed concrete and from the bottom of excavated areas. Trench <br />plates and ply wood sheets were utilized to cover the area of excavation, the area <br />was delineated with caution tape and cones. The sidewalk will be closed until the <br />site restoration is complete. Three 20 yard bins of waste concrete and excavated <br />soil were generated and transported off site under hazardous waste manifests <br />(023134177JJK, 023134173JJK, 023134172JJK) for disposal. Twenty nine <br />confirmation soil and concrete samples were collected today by Jocelyn with <br />Arcadis. <br />3:15 pm left the site to go to another spill, some left over sampling and site <br />securement still needs to be done. <br />5:20 pm, received a call from Dave Larson with PG&E, he indicated that the site <br />excavation and sampling is done and the site is secured; Dave will send pictures <br />showing the final site disposition. Site backfilling and concrete restoration will <br />occur after the confirmation sample results are received and they indicate that no <br />further excavation is necessary. <br />08/30/2021 Received update from Lead Senior Elena Manzo (EHD) regarding additional MURO <br />cleanup and confirmation samples on 8/28/2021. <br />08/30/2021 Site visit to verify spill site is still secured per Elena's request. Observed the MURO <br />caution tape was cut at each end. All other tape and metal sheets covering the <br />trench were still in place. Called Dave Larsen (PG&E) to advise that the caution <br />tape was cut at each end. Dave said he will take care of it. <br />08/30/2021 Downloaded and reviewed inspection pictures. Sent a email to Ricks, Zantha <br /><ZXS8@pge.com>; Muro, Claudia [EHD] <cmuro@sjgov.org> with a picture from <br />8/28 showing the inside of the storm drain. Some dark staining can be visible on <br />the picture located on the soil interface between the storm drain top hat and the <br />storm drain box (drain pipe) Marked the area of staining on the pictures. A sample <br />was collected above and below the observed staining, the staining was brought to <br />the attention of Jocelyn with Arcadis (while onssite), asked Zantha on what the <br />planned action for the stained soil. <br />MANZO <br />5105.rpt Page 8 of 12
