Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COGENUJ36 Site Location: IN FRONT OF 1049 S WILSON WAY <br />Activity Summary <br />Activity Date Recorded by <br />08/17/2021 Requested copy of analytical lab report for the phase one confirmation samples MURO <br />and advised to notify EHD of scheduling for phase two confirmation sampling <br />event. <br />Received response from Zantha Ricks with a copy of the 5-page Chain of Custody <br />(COC) and lab receipt listing all samples received and their conditions. <br />Zantha reported that the lab is currently overloaded with other samples and may <br />not get the results until Friday, (08-20-2021). <br />08/17/2021 Organized complaint documents. MURO <br />08/19/2021 Generated folder into Unit III "01 Pending Complaints" folder and titled it: MURO <br />COGENUJ36 PCB Spill 07-27-2021_CM. Uploaded photos from 7-27-21 and <br />8-11-2021 into Public folder: CUPA Inspection Photos and placed copy in the <br />temporary 01 pending complaints folder. Also placed copy of lab receipt and <br />coordinates to spill site in the pending complaints folder. <br />08/25/2021 Called Zantha Ricks to request sample results and scheduling information for the MURO <br />next sampling event. Left message. <br />08/26/2021 Called Alex Steele (PG&E) 209-337-8902, since I have not received an update MURO <br />from Zantha Rick's (PG&E). Alex reported that the concrete samples came back <br />non-detect for PCBs. The storm drain did come back positive. PSC contractors, <br />Dave Larson (PG&E), Arcadis, and Alex Steele will be on-site this Saturday, <br />8-28-2021 for the next phase of the sampling. They will be breaking concrete over <br />the gutter and part of the street and will be doing additional sampling. <br />Called Zantha Ricks at 925-597-7321. I let Zantha know I spoke to Alex Steele <br />regarding the lab results for the Phase 1 sampling and asked if she could provide <br />a copy of the lab report. Zantha stated that they provide a copy of the results at the <br />end of the process in the report and she if she sends us a copy, we will be <br />receiving two copies. I asked for a copy anyway. Zantha reported: <br />The crew will be on-site possibly as early as 7:30 AM to begin set-up and traffic <br />control. <br />Sampling will be done intermittently; they will be breaking concrete initially so <br />sampling will not be done immediately after the work has begun. <br />The parking lot came back non-detect. <br />In addition to the concrete/asphalt sampling, Arcadis will sample the catch basin <br />and will actively reach inside to sample the pipe. <br />Jocelyn Wong (Arcadis) will be on-site again to do the sampling (mobile #: <br />415-710-0074). <br />Point of contact will be Alex Steele as he is the lead on this spill (mobile #: <br />209-337-8902). <br />08/26/2021 Updated Elena Manzo on the information provided by Zantha Ricks. <br />Received a copy of Phase 1 EXCELCHEM Laboratories, Inc lab report. <br />08/27/2021 Received Phase 2 sampling map for the PCB sampling event scheduled for <br />Saturday, 8/28/21. <br />Called Alex Steele (PG&E) because we received a missed call from him.
