COMPLAINT INSPECTION REPORT <br />11:26 am <br />11:00 am <br />Time Out: <br />Time In: <br />Environmental Health Department <br />Name of Facility: PLAZA PERKS Date: 03/31/2023 <br />Address: 306 E MAIN ST, STOCKTON 95202 <br />Owner/Operator: PARENTS BY CHOICE INC Telephone: (209) 275-2487 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: CO0055716 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />ONE DAY LAST WEEK COMPLAINANT ORDERED A CHICKEN WRAP AND A PARFAIT. COMPLAINANT STATES THE <br />CHICKEN WAS MUSHY AND TASTED ROTTEN. ALSO STATES THE TORTILLA HAD MOLD ON IT. COMPLAINANT <br />STATES THEY HAVE PURCHASED THE WRAPS PREVIOUSLY AND NOTICED MOLD ON THE TORTILLAS. <br />COMPLAINANT ALSO STATES THE YOGURT IN THE PARFAIT WAS SOUR AND THE STRAWBERRIES WERE <br />MUSHY AND NOT FRESH. COMPLAINANT ALSO STATES THE MEAT AND CHEESE CUTS FOR SALE WERE <br />OUT-DATED. COMPLAINANT DID NOT LET THE FACILITY KNOW OF THE COMPLAINTS. <br />***COMPLAINANT REQUESTS A CALLBACK FROM THE INSPECTOR.*** <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />No Temperature Data Collected <br />FOOD ITEM -- LOCATION -- TEMP º F -- COMMENTS <br />COMMENTS AND OBSERVATIONS <br />Complaint investigation. Met with facility staff. They were unaware of the complaint. Received consent to inspect food storage <br />areas. The open-air cooler where the tortilla wraps are stored was observed at 38 F. The Silver King 2-door cooler where <br />chicken is stored was observed at 39 F. The display case where the parfait cups are stored was observed at 32 F. Inspected <br />dry storage area where tortillas are stored. Observed tortillas are within best by dates. The tortilla wraps and parfaits are <br />prepared every two days and if any of the food items remain unsold, they are discarded by the 3rd day. Food items are <br />packaged in clear containers. Did not observe any mold on any of the food items. Inspection report was emailed to the <br />operator. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
