HSC 119315 - Invoices and log kept for disposable, pre-sterilized equipment <br />OBSERVATION: Sterilization certificates were not available for the following needle cartridges: <br />Andrew: XXX lot numbers 20-09 and 21-07 <br />Armando: XXX lot # 24-07 and Cheyenne lot # 2024-37-001 <br />Mike: XXX lot # 24-01 and 23-08 <br />Anthony: Finger Ledge lot # 202403 and 202404 <br /> <br />CORRECTIVE ACTION: When using disposable, single-use, pre-sterilized instruments, the facility must maintain record of <br />written proof on company or laboratory letterhead showing that the pre-sterilized instruments have undergone a sterilization <br />process. Written proof shall clearly identify the instruments sterilized by name or item number and shall identify the lot or batch <br />number of the sterilizer run. Submit sterilization certificates to the Environmental Health Department (EHD) for lot numbers listed <br />above. <br /> <br />#7. HSC 119314 - Sharps containers labeled, used, and disposed of appropriately <br />OBSERVATION: The facility provided an invoice for the 7/30/25 pick-up. Disposal records were not available for review. <br /> <br />CORRECTIVE ACTION: Disposal records shall be maintained for 3 years and shall be available upon request. Submit the <br />missing disposal record. <br /> <br /> <br />#18. HSC 119303 - Client records approved and available - Consent form and questionnaire <br />OBSERVATION: The consent form states: “in the case of a minor receiving legal tattoo, I am the legal parent or guardian of the <br />minor receiving the legal tattoo and I am granting permission for this minor to receive the legal body tattoo:” <br />CORRECTIVE ACTION: All tattoo clients must be at least 18 years of age regardless of parental consent. This violation was <br />corrected on site. Armando updated the consent form and provided me with a copy. Per Armando, they do not offer tattoo <br />services to minors. Review of their client records confirmed all clients were at least 18 years old. <br /> <br /> <br />#33. HSC 119314 - Workstation, surfaces, including chairs, armrests, etc. in good repair <br />OBSERVATION: An absorbent cap was observed hanging on the wall in Jose’s procedure area. <br /> <br />CORRECTIVE ACTION: This violation was corrected on site. Armando removed the cap from the procedure area. <br /> <br /> <br />#35. HSC 119313 - Operation and employee training records present <br />OBSERVATION: The Infection Prevention Control Plan needs revision. <br />1. Per Armando, the facility only uses Madacide solution as disinfectant. <br />-Items A(1), A(2) and documents the use of disinfectant wipes. <br />-Item K documents bleach and disinfectant wipes as disinfectant products used at the facility. <br />2. Item C does not document storage for sterilized instruments. <br />3. Item N documents 2 sinks in the procedure area. 4 were observed on site. <br />
