The inspection report was emailed to facility owner Armando Perez, armandoperez1987@gmail.com <br />2. The facility utilizes Barnett for their sharps waste disposal. Ensure to maintain disposal records for a minimum of 3 <br />years. <br />3. Reviewed client records, medical questionnaire, after-care instructions, and pre-sterilized needle cartridges. I could not <br />access Mikayla and Jose’s client records and needles at the time of the inspection because their tool boxes were <br />locked. <br />4. Reviewed the infection prevention control plan and training record. Ensure to maintain training records for a minimum of <br />3 years. <br />5. Per Armando, there are 2 apprentices (Daniel Paniagua & Jose Espinoza) on site who are currently practicing on fake <br />skin only.
