The facility’s sterilization logs documented a temperature of 250F with <br />exposure times ranging from 30m-60m. <br />CORRECTIVE ACTION: Sterilizers shall be loaded, operated, decontaminated, and maintained according to manufacturer’s <br />directions. Moving forward, ensure to follow the manufacturer’s guide when operating the autoclave equipment. <br /> <br />#2. HSC 119309 - Items washed, disinfected, packaged, labeled, and sterilized <br />OBSERVATION: The sterilized packs are not adequately labeled. The operator’s initials are missing. <br />CORRECTIVE ACTION: Sterilization packaging must be labeled with the name of the instrument if not immediately identifiable, <br />the date sterilized, and the initials of the preson operating the sterilizaing equipment. Submit photos to the Environmental Health <br />Department (EHD) as evidence of the correction. <br /> <br />#4. HSC 119315 - Integrators used/monthly spore test/log maintained <br />OBSERVATION: Sterilization logs have incomplete dates (missing year) and some cycles are missing the exposure time (only a <br />start time was documented). <br />CORRECTIVE ACTION: Sterilization logs must include the date of the load, contents, exposure time and temperature, and <br />results of the Class V integrator. Moving forward, ensure to include the full date (month, day, year) and exposure time on all <br />sterilization logs for each cycle. <br /> <br />#5. HSC 119314 - Decontamination/sanitation area separated and supplied appropriately <br />OBSERVATION: <br />1. The sterilization room does not have a wall-mounted, touchless, paper towel dispenser. <br />2. The soap dispenser was empty. <br />3. The sink counter is damaged. <br />CORRECTIVE ACTION: <br />1. Install a wall-mounted, touchless, paper towel dispenser in the sterilization room. Submit photos to the EHD. <br />2. Supply the soap dispenser with liquid soap. Submit photos to the EHD. <br />3. Repair/replace sink counter to ensure it is smooth, nonabsorbent, and cleanable. <br /> <br />#6. HSC 119315 - Invoices and log kept for disposable, pre-sterilized equipment <br />OBSERVATION: <br />1. Client record for Stacie is missing the date of the procedure. <br />2. Sterilization certificates were not available for Precision Needles lot numbers 240825 and 240610. <br />CORRECTIVE ACTION: When using disposable, single-use, pre-sterilized instruments, the following records must be <br />maintained: <br />1. A log of all procedures, including names of the practitioner and client and the date of the procedure. Ensure to include all <br />required information on client records moving forward. <br />2. Written proof on company or laboratory letterhead showing that the pre-sterilized instruments have undergone a <br />sterilization process. Writen proof shall clearly identify the instruments sterilized by name or item number and shall <br />identify the lot or batch number of the sterilizer run. Submit the sterilization certificates for lot numbers 240825 and <br />240610.
