HSC 119314 - Sharps containers labeled, used, and disposed of appropriately <br />OBSERVATION: The facility was unable to provide disposal records for their sharps waste at the time of the inspection. <br />CORRECTIVE ACTION: Documentation of proper disposal of sharps waste shall be maintained for 3 years and shall be <br />available for inspection. Submit disposal records to the EHD. <br /> <br />#11. HSC 119314 - Handwashing facilities properly supplied and accessible, warm water <br />OBSERVATION: The wall-mounted paper towel dispensers in the procedure rooms are not fully enclosed to prevent cross <br />contamination. <br />CORRECTIVE ACTION: Provide fully enclosed wall-mounted paper towel dispensers for all handwash stations. Submit photos <br />of the EHD. <br /> <br />#32. HSC 119314 - Floors and walls clean and in good repair, adequate light <br />OBSERVATION: Holes were observed on the walls outside the blue room and inside the blue, yellow, and red rooms. <br />CORRECTIVE ACTION: Repair and repaint holes with gloss finish paint. Submit photos to the EHD. <br /> <br />#33. HSC 119314 - Workstation, surfaces, including chairs, armrests, etc. in good repair <br />OBSERVATION: The procedure chair in the blue room is missing 2 armrest fittings (one on each side) exposing the absorbent <br />cushion. <br />CORRECTIVE ACTION: Repair/replace the procedure chair to ensure all surfaces are smooth, cleanable, and non-absorbent. <br />Submit photos to the EHD. <br /> <br />#35. HSC 119313 - Operation and employee training records present <br />OBSERVATION: The Infection Prevention Control Plan’s (IPCP) sterilization procedures do not follow the manufacturer’s <br />manual. <br />CORRECTIVE ACTION: Update the IPCP’s sterilization procedures in accordance to manufacturer’s instructions and provide <br />training to all employees. Submit the updated IPCP and training records to the EHD. <br /> <br /> <br /> <br /> Submit evidence of corrections to the EHD by <br />07/30/2025 <br /> <br /> <br /> <br />Notes: <br />1. Report emailed to facility owner Sintia Mejia at sintiamejia@yahoo.com <br />2. 5 piercing rooms inspected (Blue, Yellow, Black, Green, & Red rooms). Previously approved tattoo room is now used for <br />nail services. The other rooms in the facility are utilized as a prepackaged snack store (World Cravin’), office, storage, <br />sterilization room, and break room. <br />3. Reviewed client consent forms, medical questionnaire, and after-care instructions. <br />4. Reviewed the Infection Prevention Control Plan (IPCP) and training log. <br />5. Reviewed sterilization logs and monthly spore testing logs. <br />6. Reviewed jewelry invoices to verify material grade: 14k gold and AST F138 verified. <br />7. The facility utilizes Sharps Assure mail-back system for their sharps waste disposal. <br />8. If the facility does not intend to continue offering tattoo services, the IPCP needs to be updated. <br />9. Per Sintia, all other practitioners are no longer at this facility. <br />10. Complaint #CO2500345 allegations regarding improper sharps management was addressed during the inspection.
