Each sterilization load shall be monitored with mechanical indicators for time, temperature and pressure, Sterilization shall be <br /> accomplished by steam autoclave, manufactured for the sterilization of medical instruments, and pass a Class V integrator test. <br /> HSC 119315(b)—The Crosstex integrators are not Class V integrators. Ensure the integrators used are Class V <br /> integrators. <br /> 3. Sterilizer shall be loaded, operated, decontaminated, and maintained according to manufacturer's recommendations. HSC <br /> 119315(b)—Evidence of calibration of the autoclave was not provided to the EHD. Calibrate the autoclave and provide <br /> evidence to the EHD. <br /> 7. The sharps waste container shall be within arm's reach and labeled with the word "sharps waste"or with the biohazard symbol <br /> and the word "Biohazard". Sharp waste containers shall be disposed by a licensed waste hauler or approved mail back system. <br /> Dccumentation of proper disposal shall be maintained for 3 years. HSC 119314(e)—A manifest for sharps waste disposal <br /> wzs not provided to the EHD. Provide evidence of proper sharps waste disposal to the EHD. <br /> 18 The facility's client consent form and medical questionnaire must include all of the information listed in Form B, and are <br /> available for inspection. HSC 119303—Three years of client records were not at the facility. Three years of client records <br /> should be at the facility for review by the EHD. <br /> 25 Disinfectant shall be used according to the manufacturer's instructions. HSC 119309(b)—The wet contact time for the <br /> Madacide spray provided by the practitioner was 30 seconds. Per manufacturer's instructions,the wet contact time for <br /> eliminating HIV and Hepatitis B is 6 minutes. Ensure manufacturer's instructions are followed. <br /> 32 A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br /> The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br /> floor, from na'I and hair activities and be separated from all business not related to body art. Approval of co-located body art <br /> facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br /> procedure area shall have lined waste containers. HSC 119314 & 119317—The following was observed in the procedure <br /> rooms: <br /> Blwe Room—Holes in the wall,gap in between the walls and the door frame is in need of repair. <br /> Ye4low Room—Holes in the wall (above the counter on the white flower petal). <br /> Repair/seal the hole and the gap. Provide EHD with photo graphic evidence of correction. <br /> 35. Operation and employee training records shall be maintained for 3 years and made available for inspection. A written <br /> Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313—Update the <br /> information on Section G and Section T of the IPCP to include "tattoo" artist. Update licensed waste hauler information <br /> with mailback system on the IPCP. Train all practitioners on the IPCP after update is complete. Provide a copy to the <br /> EHD. <br /> Rei ispection on/about: A reinspection fee of$162 per hour may be charged.
