A body art facility shall notify the LEA in writing within 30 days of resignation, termination of a <br /> practitioner.A new hire or apprentice must be registered the EHD prior to performing body art at the facility. HSC 119312 & <br /> 119306—EHD was informed Estrella Pantoja and Ingri Castillo Martinez are not practicing at the facility. EHD was not <br /> notified within 30 days. Ensure EHD is notified in writing of any practitioner changes. <br /> Correct all violations and provide EHD evidence of corrections <br /> by September 28, 2023. <br /> Notes: <br /> 1. Currently, there are six(6) rooms at the facility that are permitted by the EHD for body art. <br /> 6.
