Prior to the performance of body art, the client shall read, complete, and signed and informed consent form. On the <br />consent, a statement that depicts that tattoo inks, dyes, and pigments have not been approved by the federal Food and <br />Drug Administration and that the health consequences of using these products are unknown. HSC 119303(a) and (a)(4) <br /> <br />OBSERVATION: <br />Client consent forms were not being fully completed by the client or the practitioner. One of the client consent forms did not have <br />all of the required client signatures. There were procedure description forms that were not being completed. <br />1. Client consent forms were not being fully completed by the client. Signatures from the client were missing on the forms. <br />2. Description of the procedure form was not being filled out. <br />3. Practioner signatures were missing on the client consent forms. <br />4. No notice on the client consent forms that tattoo dyes and inks are not approved by the FDA <br /> <br />CORRECTIVE ACTION: <br />1. Assure that the client consent forms are being completed in full. <br />2. Assure the description of procedure form is being completed in full. <br />3. Assure the practitioner who is performing the body art is also signing all necessary documents to ensure that the client <br />consent forms are being fully complete. <br />4. Update the client consent to include that the FDA does not approve of tattoo dyes and inks and that they health <br />consequences are unknown. <br /> <br />24. A facility that performs body art shall use a an appropriate disinfectant which is a product that is registered by the <br />federal EPA and the Department of Pesticide Regulation, as indicated on the label, to reduce or eliminate the presence <br />of disease-causing microorganisms, including HIV and HBV for use in decontaminating work surfaces. HSC 119301(k) <br /> <br /> OBSERVATIONS: <br />The disinfectant provided for review was Lysol disinfectant spray. Upon review of the product, it does not adhere to regulation in <br />eliminating HIV and HBV. <br /> <br />CORRECTIVE ACTION: <br />Provide proof of purchase of an appropriate disinfectant that eliminates presence of HIV and HBV. <br /> <br />32. A body art facility shall comply with having floors and walls that are smooth, nonabsorbent, and free of open holes, <br />and washable. HSC 119314(a)(2) <br /> <br />OBSERVATIONS: <br />The walls of the procedure area had patches of plaster did not feel like they were easily washable and nonabsorbent. <br /> <br />CORRECTIVE ACTIONS: <br />Please provide a finish to the walls that are smooth, non, absorbent, and easily washable. <br /> <br />33. A body art facility shall have all counter surfaces and treys shall have a smooth, durable, and non absorbent finish. <br />HSC 119314(b)(6) <br /> <br />OBSERVATIONS: <br />1. A lamp shade that was within the procedure area was made of a felt material that was absorbable. <br />2. A throw pillow on the chair to the right of the hand washing sink was made of cloth material making it absorbable. <br /> <br />CORRECTIVE ACTIONS: <br />Remove any and all decorations from the procedure room that are made of an absorbable material. <br />
