A body art facility shall have its health permit posted in a conspicuous place. Certificates of registration for all <br />practitioners performing body art in the shall also be prominently displayed either near the health permit or at the <br />infividual station. <br /> <br />OBSERVATION: <br />No health permit was posted any where in the facilty. <br /> <br /> <br />CORRECTIVE ACTION: <br />Please post Environmental Health permit within the facilty so as to be easily viewed by enforcement officer. <br /> <br />35. A body art facility shall have a written Infection Prevention Control Plan, and have records of training and shall be <br />maintained for three years and shall be available for inspection upon request of the enforcement officer. <br /> <br />OBSERVATION: <br />Facility did not have an IPCP on site for review. <br /> <br />CORRECTIVE ACTION: <br />Please maintain a copy of the IPCP on site for review. If any changes are to be made to the IPCP, then an updated copy of the <br />IPCP must be sent to the Environmental Health Department. <br /> <br /> <br />Correct all violations and provide evidence of corrections <br />to EHD by March 18th 2025. <br /> <br /> <br />Notes: <br />1. Inspection report emailed to tuep444@gmail.com <br />2. IPCP was not available on site for review. <br />3. Facility offers microblading to clients <br />4. Health permits and BBP certificate were not posted in the facility. <br />5. Only 3 client consent forms were provided for 2023-2024. Per operator, the facility rarely gets any clients which is the <br />reason for the low amount of client consent forms. <br />6.
