The following is an itemized list of underground storage tank violations that have not <br /> been addressed for KETTLEMAN CHEVRON as of December04, 2023. <br /> Open violations from August 28, 2023 inspection <br /> Violation#104-Failure to submit a complete and accurate UST Monitoring Plan. <br /> OBSERVATION The monitoring plan is not current. The persons listed as having responsibility on the diesel, 87 <br /> (tank#3), and 91 monitoring plans are outdated. <br /> REGULATION GUIDANCE: The monitoring plan must be uploaded to the California Environmental Reporting <br /> System (CERS). <br /> CORRECTIVE ACTION: Immediately log into CERS, make the necessary changes, and submit for review by the <br /> EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#107 -Failure to submit and maintain complete and current financial responsibility documents. <br /> OBSERVATION Financial responsibility documents are incomplete. The letter of chief financial officer letter did not <br /> include the facility name and address and the location that the letter was executed. <br /> REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the required documents, and maintain a copy on site. <br /> ❑ This violation was corrected ❑ This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#109 -Failure to submit"Designated Underground Storage Tank Operator Identification Form" <br /> within 30 days. <br /> OBSERVATION The designated operator identification form has not been submitted in CERS. <br /> REGULATION GUIDANCE: The owner or operator shall submit a"Designated Underground Storage Tank Operator <br /> Identification Form" no later than 30 days after any change in the designated UST operator(s). <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br /> Page 1 of 3 <br />
