The following is an itemized list of underground storage tank violations that have not been addressed for KETTLEMAN CHEVRON as of December04, 2023. Open violations from August 28, 2023 inspection ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#203 -Failure to maintain monitoring records for 36 months and maintenance records for 60 months. OBSERVATION Monitoring records for the last three years were not found on site. The 2022 monitoring system certification and spill container test results were not found on site. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site. . ❑ This violation was corrected ❑ This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #205 -Failure to submit and have current UST Monitoring Plan readily accessible at the facility. OBSERVATION An approved copy of the monitoring plans were not available for inspection. REGULATION GUIDANCE: A copy of these plans shall be accessible on site at all times.
