Exhibit B <br /> CONDITIONS OF APPROVAL <br /> MOUNTAIN HOUSE INVESTORS,LLC/Gerry Kamilos/Tract No.4098 <br /> Major Subdivision Application No. PA-2300148 was approved by the Board of Supervisors <br /> on . The effective date of approval is . These tentative map approvals will <br /> expire on , which is 3 years from the effective date of approval, unless (1) all <br /> Conditions of Approval have been complied with and (2) a Final Map has been filed with and <br /> accepted by the County Surveyor. <br /> Unless otherwise specified, all Conditions of Approval and ordinance requirements shall be <br /> complied with prior to approval of the Final Map. Those Conditions followed by a Section <br /> Number have been identified as ordinance requirements pertinent to this application. <br /> Ordinance requirements cannot be modified and other ordinance requirements may apply. <br /> 1. COMMUNITY DEVELOPMENT DEPARTMENT (Contact John B. Anderson: [209] 468- <br /> 3121) <br /> GENERAL <br /> a. Tentative Map Compliance: The Final Map shall be in substantial compliance with the <br /> Tentative Map dated March of 2023 by MacKay & Somps Civil Engineering (attached as <br /> Exhibit A), and all Conditions of Approval. <br /> 1. The project shall comply with applicable policies of the Mountain House Master Plan <br /> and all other applicable Community Approvals. <br /> 2. Minor modifications to street and lot configurations may be allowed by the Community <br /> Development Director without requiring a new subdivision map application or a <br /> Revision of Approved Actions application. <br /> 3. Secondary housing units are required to be designed and built within 10% of the homes <br /> built within this project. This project has designated 1 lot, Lot 2, for the construction of <br /> a secondary unit as indicated on the Tentative Map. The Community Development <br /> Director may allow the location of the secondary units to be adjusted though the <br /> Building Permit process. <br /> Timing.Prior to Final Map approval and any other Development Approvals subsequent <br /> to the Tentative Map. <br /> b. Subsequent Approvals: All subsequent discretionary and ministerial approvals shall be <br /> subject to Design Consistency Review (DCRC) and shall be consistent with the conditions <br /> of approval for the Tentative Map, Pursuant to Chapter 9-205.8M of the County <br /> Development Code. The Final Map shall be recorded containing the following note: <br /> "All subsequent discretionary and ministerial approvals within the boundaries of this <br /> subdivision map shall be subject to Design Consistency Review (DCRC) and shall be <br /> consistent with the conditions of approval adopted for the Tentative Map." <br /> Resolution Template 01/2023 <br />
