(3) INFORMED CONSENT FORM <br /> (a) Informed consent form for each client shall be read,completed,and signed prior to any procedure being performed and <br /> shall include all elements of section 119303 of the Safe Body Art.2 <br /> (b) The Informed Consent forms(including consent,post-procedure instructions&medical questionnaire)must be main- <br /> tained in a secure location on-site and available for review during permit inspections for a period of two years. <br /> (c) After two years from performing the body art procedure,the body art facility shall shred any confidential medical <br /> Information from the client. <br /> (4) POST-PROCEDURE INFORMATION <br /> (a) Printed procedure-specific instructions on the care of the procedure site shall be given to each patron or customer after <br /> tattooing, branding, or piercing as a precaution to prevent infection. Instructions shall include restrictions on physical <br /> activities such as bathing, recreational water activities, gardening, or contact with animals and the duration of the <br /> restrictions, signs, and symptoms of infection, and indicate the need to seek medical care. Each printed Home-Care <br /> Instructions shall contain the Business name,address,and Ph number. <br /> (b) Signs and symptoms of infection including but are not limited to,redness, swelling,tenderness of the procedure site,red <br /> streaks going from the procedure site toward the heart,elevated temperature,or purulent drainage on the procedure site. <br /> (5) DYES OR PIGMENTS <br /> (a) In preparing inks, dyes or pigments to be used by a tattoo artist, only commercially available single-use or individual <br /> portions of dyes or pigments in clean, single-use containers shall be used for each patron. <br /> (b) After tattooing, the remaining unused dye or pigment in the single-use individual containers shall be discarded along <br /> with the container. <br />
