(i) If the design is drawn directly onto the skin,it shall be applied with a single-use article only. <br /> (j) Cabinets for the storage of instruments,dyes,pigments,and single-use articles,shall be provided for each artist and shall <br /> be maintained in a sanitary manner,which protects the instruments from contamination. <br /> (k) Needles, needle bars, grommets, and razors shall be discarded into the sharps waste container immediately upon <br /> completion of the procedure. <br /> (1) The surface of all work tables and chairs or benches shall be constructed of a material that is smooth,nonabsorbent,and <br /> easily decontaminated. <br /> (m) Worktables/trays and chairs or benches shall be decontaminated with a germicidal solution after each tattoo application. <br /> Hard surface disinfection will consist of <br /> Madacide and/or Bleach wipes <br /> All hard surfaces in the procedure area will be disinfected after each client. The chaos,counters,etc.will be sprayed <br /> with: <br /> Madacide.Caveside.and/or Bleach wipes. <br /> The disinfectant will be allowed a contact time of Between 10-15 minutes or:(according to manufachirer's <br /> directions for dilution and contact time), <br /> then wiped down with a paper towel which is disposed of in the trash. <br /> (n) All materials applied to the human skin shall be from single-use dispensers transferred from bulk containers to single- <br /> use containers and shall be disposed of after each use. <br /> (o) No animals shall be permitted in the procedure area. <br /> (2) RECORDS <br /> (a) Records,including a complete informed consent forth and post-procedure instructions for all tattoo, brand,and piercing <br /> administered, shall be maintained by the holder of the permit. All records will be maintained on-site and available for <br /> review for a period of:Two(2)Fears,after which tile records shall be shredded to destroy confidential <br /> medical information. <br /> (b) A copy of all registrations, licenses, and Exposure Control Training for all body artists employed or contracted to the <br /> facility will be maintained on-site,and available for review. <br /> (c) Records and procedure logs of all purchased pre-sterilized instruments/parts will be maintained on-site,and available for <br /> review for a period of: <br /> 90 days after the instrument is used. <br />
