HSC 119315 - Invoices and log kept for disposable, pre-sterilized equipment <br />OBSERVATION: Sterilization certificates for the EOS needle cartridge lot #2024-01 was not available at the time of the <br />inspection. <br />CORRECTIVE ACTION: Submit a copy of the missing sterilization certificate to the Environmental Health Department (EHD). <br /> <br />#11. HSC 119314 - Handwashing facilities properly supplied and accessible, warm water <br />OBSERVATION: The practitioner’s restroom inside the facility does not have a touchless paper towel dispenser. <br />CORRECTIVE ACTION: Install a wall mounted, touchless paper towel dispenser inside the practitioner restroom. Submit photos <br />to the EHD. <br /> <br />#18. HSC 119303 - Client records approved and available - Consent form and questionnaire <br />OBSERVATION: Client consent forms are inadequate. The consent forms are missing the description of the procedure and the <br />description of what the client should expect following the procedure. <br />CORRECTIVE ACTION: Ensure the client consent forms include the missing elements along with all items listed in HSC 119303 <br />and submit a copy to the EHD. <br /> <br />#32. HSC 119314 - Floors and walls clean and in good repair, adequate light <br />OBSERVATION: The floor in the procedure area is damaged and is currently covered with a nonabsorbent mat. This item was <br />previously documented during the last inspection and has not been resolved. <br />CORRECTIVE ACTION: The damaged flooring needs replacement. Submit proposed plans of floor replacement for approval <br />prior to completing the work. Ensure floors are smooth, nonabsorbent, free of open holes, and washable. <br /> <br />#35. HSC 119313 - Operation and employee training records present <br />OBSERVATION: The Infection Prevention Control Plan (IPCP) is inadequate. It does not include all required elements listed in <br />HSC 119313. The IPCP is missing procedures for storing reusable instruments, procedures for protecting clean instruments and <br />sterile instrument packs from exposure to dust & moisture, set-up and tear-down procedures, and techniques to prevent <br />contamination of instruments or the procedure site, <br />CORRECTIVE ACTION: Ensure the IPCP contains all required items listed in HSC 119313 and submit the revised IPCP to the <br />EHD. <br /> <br /> <br /> <br />Submit evidence of corrections to the EHD by <br />04/26/2025. <br /> <br /> <br /> <br />NOTES: <br />1.
