The following is an itemized list of hazardous materials violations that have not been <br /> addressed for AAMCO TRANSMISSION as of December 19, 2023. <br /> Open violations from October 30, 2023 inspection <br /> Violation#1 -Failed to pay annual operating fees. <br /> OBSERVATION: Facility failed to pay annual operating fees. The facility has a balance of$4,489.26. <br /> ORDINANCE: Fees shall be paid once a year at the time of submittal of an inventory form. <br /> CORRECTIVE ACTION: Immediately pay the annual fee to the EHD Accounting Office located at 1868 E Hazelton <br /> Avenue, Stockton, CA 95205. Fee may also be paid online at https://www.sjgov.org/department/envhealth/fees <br /> (Convenience fee may apply). <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#3-Failed to establish and implement a HMBP. <br /> OBSERVATION: Per Vincent Kemp Vanee-Manager, he is unaware of what the Hazardous Materials Business <br /> Plan is.A Hazardous Materials Business Plan has not been implemented. <br /> REGULATION GUIDANCE: A Hazardous Materials Business Plan (HMBP)shall be established and implemented <br /> for emergency response to a release or threatened release of a hazardous material when storing hazardous <br /> materials at or above the thresholds quantities of 55 gallons for materials that are liquids, 500 pounds for solids, or <br /> 200 cubic feet for compressed gas, at standard temperature and pressure. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the required information, and submit for review by the EHD./Submit a <br /> statement/documentations to SJC EHD demonstrating that the HMBP has been established and implemented. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#9-Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15.The business plan (facility <br /> information and chemical inventory)was last submitted on March 27, 2019.The Emergency response and training <br /> plan was last submitted on March 14, 2019. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> Page 1 of 3 <br />
