If contaminated sharps (e.g., needles, metal or glass) or other contaminated material is an expected/potential hazard at <br /> a project site, the HASP will include instruction for its removal by a professional company/service. If such items are <br /> unexpectedly found at a site, Stop Work, isolate the area in question, contact the client and discuss with the client <br /> options for contracting with a professional company service for cleanup and removal. <br /> Hepatitis B Vaccination Series <br /> Employees who are designated to render first aid as part of their job duties will be offered the Hepatitis B vaccination <br /> series after they've received the required BBP training. The vaccination series is offered without cost to the employee <br /> and consists of three injections. <br /> Employees who decline the infection will be asked to sign a Hepatitis B Declination form <br /> (Appendix 1-from the Arcadis Bloodborne Pathogens Exposure Control Plan Template) <br /> which will be placed in the employee's Corporate Health and Safety file. If an employee later decides to have the <br /> vaccination and is still designated to provide first aid, the vaccination will be made available at no cost to the employee. <br /> Post Exposure Evaluation and Follow-Up <br /> it an employee teeis tnat ne/sne nas had an exposure to a t bF, the employee will oe sent for a medical evacuation it the <br /> employee so chooses. The employee's supervisor or a manager will complete the medical provider information form <br /> (Attachment 1) and send it with the employee to the evaluation. <br /> The employee will receive a detailed report from the medical provider, and Arcadis will receive notification that the <br /> employee has been informed of the results of the evaluation and any recommended treatment. <br /> The incident will also be reported to Corporate Health and Safety. <br /> Training <br /> All employees who are designated to render first aid will be given BBP training upon initial designation and annually <br /> thereafter. The training will be provided before the employee is offered the Hepatitis B vaccination. <br /> Record Keeping <br /> Medical records regarding an exposure to a BBP and Hepatitis B vaccination information will be maintained by the <br /> healthcare provider. Records provided to Arcadis will be maintained by Corporate Health and Safety permanently, and <br /> will be kept confidential and not disclosed without an employee's written consent except as required by law. <br /> BBP exposure training records will be maintained in the Arcadis training database. <br />
